Purpose of Signature Assignment:
Signature assignment requires students to demonstrate and apply their proficiency in one or more
key learning outcomes. This often means synthesizing, analyzing, and applying cumulative
knowledge and skills through problem-or inquiry-based assignments or projects. This design
invites students to produce meaningful examples of their learning that document their progress
and culminating skills and abilities.
Purpose of the Assignment
The purpose of this activity is to evaluate, in the context of one patient encounter, the full
spectrum of primary health care services to include health promotion disease prevention, health
protection, anticipatory guidance, counseling, disease management, palliative and end of life
care. Additionally, evaluate the results of interventions using accepted outcomes criteria, revise
the plan accordingly, consult/refer when needed, and perform peer review of outcomes.
Standards & Learning outcome
This course meets the AACN Essentials of Masters Education for Advanced Practice Nursing
(2011) In addition the course incorporates the NONPF Nurse Practitioner Core Competencies
(2017) and the Population-Focused Nurse Practitioner Competencies for Family/Across the
Lifespan (2013).
1. Learning outcomes:
a. Students will utilize standards of care in health promotion and anticipatory
guidance.
b. Students will evaluate management of disease processes using evidence-based
guidelines.
c. Students will evaluate the results of interventions and describe how to revise plans
for continuous improvement.
Directions:
A. Student will select an actual patient case and address all 6 core competencies as described
in Quality and Safety Education for Nurses (QSEN).
B. The following questions need to be addressed:

If so, how was this was done? Were patient wishes carried out? How do you
know? Were there cultural considerations? Were barriers to care identified?

collaboration with the preceptor. What communication strategies worked well?
What could have been improved?
Evidence Based Practice: What reputable evidence was used to formulate plan
of care for this patient? How did you know it was appropriate for this specific
patient?
Continuous Quality Improvement: How could this encounter have been
improved for the patient? What went right for this patient? What could have been
done differently?
Safety: What risks are identified for this patient and what is a reasonable way to
address them today? What steps need to be taken regarding medication
reconciliation with this patient? What places this patient at highest risk? What can
go wrong? What could you have done to further address this issue?
Informatics: What types of electronics (EHR, data bases) were used for this
patient? Was the technology helpful? How so? What additional form of electronic
information could have been useful to you or would have been helpful to your
patient?
Plan for Improvement: Reflects on a plan on how to improve practice, when
faced with a similar situation.
C. Submit a maximum of 5-page paper addressing all the questions in appropriate APA
format with correct grammar, usage, and mechanics as expected of a nursing graduate
professional.
D. Each student will access the appropriate research literature that includes scholarly
periodicals (journals) that are peer-reviewed, and that are prepared specifically for a
scholarly or professional audience. At least three reference sources in APA format
from the scholarly periodical (journal) literature must be included. Other sources,
such as textbooks, electronic resources such as web sites, and publications for the lay
public may be included, but are in addition to the scholarly periodical references
required.
E. Include a title page and an APA-format abstract that briefly describes the content of
your discussion (see grading rubric).
